OET Speaking: Role-playing In The Covid-19 Crisis

INTRODUCTION

The OET or Occupational English Test is an English language proficiency test for practitioners of health professions, looking to register and practice in primarily English-speaking countries, or for academic purposes. In the current climate, with Covid-19 creating worldwide panic and uncertainty, the ability of healthcare professionals to communicate with people over such a crisis, is paramount.

OET Speaking Test: Role-Play

The OET speaking test is designed to test your ability to communicate via speaking, in a professional context. 

The test is profession-specific, in that you will be given tasks specific to one of the 12 healthcare professions covered by OET, as per your registration. This is based on the understanding that each profession has specific communication requirements.

The test is conducted by an interlocutor, individually. It begins with brief introduction and conversation. 

The main tasks are in the form of role-plays.

In the role-play, you will play the role of the healthcare professional, while the interlocutor plays the role of patient/client/patient’s relative or carer. 

You will be given a card, explaining the situation, which is based on realistic workplace situations. You will be given 3 minutes to prepare. There are 3 tasks in total.

Criteria For The Speaking Test

The criteria, based on which are tested, consists of 2 categories – Linguistic and Clinical.

Linguistic criteria include those related to using the language – including intelligibility, fluency, appropriateness of language and resources of grammar and expression.

These criteria indicate that you will make sense while speaking to your patients or any other person in a professional environment. From their perspective, clarity in communication is essential in difficult situations, and are required to help them build and maintain trust in you as their health care practitioner.

 

Clinical criteria, as described below, are arguably even more important – particularly in the context of a pandemic, as we are seeing with Covid-19 in 2020.

Keep the following role-play situation in mind – “A patient has come for a consultation because she believes that she has symptoms of coronavirus, and wants to know what to do.”

 

Clinical Criteria

Relationship-Building

This involves introducing yourself and greeting your patient, displaying an attentive, respectful and non-judgmental attitude, and showing empathy for your patient.

 

Maintain a calm demeanour. Your approach must show that the issue is serious, but that the patient is in a safe space. 

Information Gathering 

You will ask your patient about what she has to say about the situation. Your approach must reflect the following:

  • Listen actively, and minimize interruption.

  • Ask open questions (e.g. how are you feeling?) and then move to closed questions (e.g. yes/no).

  • Do not use complicated, long-winded questions, or leading questions (which might lead the patient to say something that she does not mean).

  • Clarify her statements which are vague or need further explanation 

When she is done speaking, summarise what she has said, asking her if she would like to make any corrections, or add anything more.

 

Ask the patient the appropriate questions regarding symptoms, time of onset etc.

 

When asking questions regarding why the patient believes she has the virus, or what measure she has taken to limit the spread to other people – maintain a non-judgmental attitude.

 

Avoid using words that might create panic, such as ‘pandemic’. Treat the patient as an individual whose concerns matter.

Understanding & Incorporating The Patient’s Perspective 

Allow the patient to explain her ideas, concerns or expectations. 

You will have to pick up on her cues, verbal and non-verbal.

You will accordingly make your explanation in relation to what she has said.

 

In a situation like Covid-19, the patient will almost definitely have read a lot of information and even misinformation before meeting you. Make sure that you address and clarify everything she says/asks, without dismissing them.

Information Giving 

This refers to your approach when you have to give the patient information or advice:

  • Begin by establishing what she already knows 

  • Pause periodically while giving information. Use her responses to help you continue

  • Encourage her to voice her own reactions or feelings 

  • Check whether she has understood the information 

  • Ask about any further information she needs

 

Be honest about her condition and what is going to happen.

 

Be clear regarding the procedures the patient has to follow thenceforth. 

 

Ensure that she understands the seriousness of the issue, without attempting to cause fear.


 

Providing Structure 

Speaking in a logical and purposeful way allows for clarity and trust-building. Organise your explanation, rather than speaking in a haphazard manner.
